The Andrew J. Applicants must be residents of Massachusetts who have been honorably discharged from service. Apart from completing the application, candidates must also: provide two letters of recommendation from individuals (not related to you) who are in a position to evaluate your character, commitment, and capacity to succeed in your future endeavors; provide a copy of your DD 214 showing your honorable discharge from service; provide proof of payment to your selected college or technical school; and complete a personal reflection essay of about 500 words (1-2 pages) describing how your service has impacted you personally and how you will continue to serve your community or country in the future (you should also describe your educational and professional goals for when you complete your education).

The Andrew J. Zabierek Foundation was formed in 2004, following the death of Lance Corporal Andrew J. Zabierek, a Marine from Chelmsford, MA. Lance Corporal Zabierek was killed in action in the Al Anbar province of Iraq while responding to a mortar attack outside his camp. He was posthumously awarded the Purple Heart and was honored by the town of Chelmsford with their Guardian Award in May of 2007. He was also awarded The Medal of Valor by the Chelmsford Elks in June of 2008. In an effort to create something positive from his passing, members of his family and community leaders created the foundation which bears his name in the hope of continuing the legacy of sacrifice and service that Andrew's memory inspires.
