To be eligible for the John E. Kostic Memorial Foundation Juvenile Diabetes Scholarship, applicants must: be a high school senior with type 1 diabetes seeking a higher education at an accredited four year university, college, technical or trade school; be a New Jersey resident; participate in community or extra curricular activities or sports; demonstrate good academic performance; and have an endorsement from a physician or CDE that demonstrates that they are managing their diabetes (HbA1c is not a criterion).
Besides completing the application, applicants must also submit: proof of citizenship (via a copy of either a state ID or a US passport); a transcript; a document listing their involvement in extracurricular activities; a letter of recommendation from a teacher, coach or counselor; and an essay (500-word limit) explaining the importance of their prospective major in today's society. The winners will be notified via email no later than July 1. Upon verification that the winners have been accepted and registered to an accredited four year university, college, technical or trade school, the checks will be distributed in July directly to the school. The number of recipients and dollar amounts will vary each year.
