Anyone 17 or older, who is a resident of the United States and who has been diagnosed by a physician as having any form of rare disease, regardless of treatment status. Applicants will be asked to provide a Diagnosis Verification Form provided by a physician verifying diagnosis.
Eligibility Criteria:
- Must be aged 17 or older
- United States resident
- Diagnosed with any form of rare disease
- Must plan to enroll in and accredited college, university or trade school in the coming academic year
Application Requirements:
- Applicants need to provide a Diagnosis Verification Form from a physician
- Undiagnosed individuals may apply with a completed Diagnosis Verification Form
- A disease is considered rare if it affects fewer than 200,000 people in the United States.

If selected as a recipient, the student will receive a $5,000 award. Traditionally, up to 35 awards are granted each year. Thanks to additional funding provided by Amgen, 88 awards will be provided for the coming fall. Interested awardees may have the opportunity to share their story with Amgen or on EveryLife social media, newsletters, or website. Meetings and publicity are completely optional.
