To be eligible for the Salix Gastrointestinal Health Scholars Awards, applicants must: be a United States citizen living in the United States; have applied to, have been accepted to, or are currently attending an accredited, nonprofit, two- or four-year college, university, or advanced (post-high school) vocational or technical school for the upcoming academic year; have been diagnosed with and treated for a gastrointestinal condition; and have been treated by a licensed healthcare provider. These awards are open to undergraduates, graduate students and parents pursuing undergraduate, vocational/technical or graduate degrees. Applicants must be US citizens.
Apart from completing the application, applicants must also submit: two letters of recommendation and information about their school and community activities; a signed form confirming their gastrointestinal condition by the treating licensed healthcare provider; proof of application or acceptance to, or enrollment in, the school for which they are seeking the scholarship; and an essay (500 words) describing the impact their gastrointestinal condition has had on them, and the role their healthcare provider has played in helping treat the condition.
