The following is a list of the qualifications and requirements:
• Michigan Resident
• Confirmation of enrollment in college or trade school
• Confirmation of asthma/allergy diagnosis
• Provide insight (in essay form) into your background of how asthma/allergies have influenced your life and how
you've overcome any obstacles from this diagnosis.
