The Thomas J. Seefred Scholarship was established in 2005 with funding generated by the Thomas J. Seefred Trust. Its purpose is to award scholarships for college students between the ages of 18 and 25 who suffer from juvenile diabetes and are pursuing a bachelor's degree.
-Available only to Ohio residents
-Must be 18-25 years old at time of application
-Diagnosed with juvenile diabetes
-Successfully completing a standard curriculum leading to a bachelor's degree at an accredited junior college, college or university
-Applicant must be attending college full-time
-Must be in good academic standing
-Proof of enrollment: an officially dated transcript from the college indicating current enrollment and GPA
-Physician's statement on letterhead noting diagnosis and treatment
-One (1) letter of recommendation from a non-relative
-Personal essay (500 words or less describing the impact juvenile diabetes has had on you and why you feel you should receive this scholarship
-Current income tax statements for parents and student
-The recipient shall be required to maintain records of expenditures and proof thereof - *Please provide 4 copies of all documents
