Scholarship Description

Nephrology social worker, Beth Witten, established the Culpepper Exum Scholarship as a memorial to her brother who had diabetes and was on dialysis prior to his death from heart disease. One scholarship is awarded to a Kansas resident and one to a Missouri resident each year.

Eligibility Criteria

  • Must be a resident of Kansas or Missouri
  • Must be on dialysis, a kidney transplant recipient, or in stages 2 through 5 of kidney disease
  • Must be pursuing post-secondary education or career training
  • Scholarship may be used at an accredited trade school, continuing education program, college, university, or vocational program
  • Applicants unsure of their kidney disease stage should consult their healthcare team

Application Process

  1. Obtain the scholarship application through your social worker or download the electronic form
  2. Complete the application with all required information and documentation
  3. Confirm your residency in Kansas or Missouri and your eligibility as a dialysis or transplant recipient
  4. Submit your completed application by email or mail
  5. Ensure mailed applications are postmarked no later than May 11th

This scholarship is intended to help two deserving people on dialysis or transplant recipients to attend any accredited post-secondary educational or vocational program. The award is for up to $1,000 per scholarship paid directly to the institution. Two scholarships will be awarded per school year—one to a Kansas resident and one to a Missouri resident.
