The Trilogy Health Services Foundation (The Foundation) External Scholarship Program is available to current Kentucky, Ohio, Indiana, Michigan and Wisconsin high school seniors or rising collegiate freshmen who intend to pursue a health-related field and desire to work with the geriatric population.
Scholarship Details
- Two $2,500 scholarships awarded annually in each Trilogy state
- Scholarship renewable for up to four years
- Open to high school seniors or rising collegiate freshmen pursuing a health related field
- Focus on students interested in working with the geriatric population
Eligibility Criteria
- Applicant must live in Kentucky, Ohio, Indiana, Michigan, or Wisconsin
- Applicant may attend college in any state
- Applicant must have a minimum 3.5 GPA
- Applicant must be in good standing with their university to receive recurring funds
- Applicant must submit an updated resume and transcript each year for renewal consideration
Application Process
- Confirm eligibility based on residency, academic standing, and intended field of study
- Complete the scholarship application during the January 1st to March 1st submission period
- Provide answers of 250 words or less to the required short answer questions

The Foundation will grant two $2,500 scholarships per state (KY, OH, IN, MI, WI) per year for four years.
